These screenings are for JHS employees ONLY. You will be screened for:

• BMI
• Waist Circumference
• Blood Pressure
• Blood Sugar
• Cholesterol
• Tobacco Use

IMPORTANT REGISTRATION INFORMATION:

On April 12th and 13th, essential and non-essential employees will be screened in different rooms. If you register for an appointment on one of those days, make sure you sign up for the correct room by selecting the offering that says "Essential Employees" or "Non-Essential Employees" based on what is applicable for you. You can also use the "Filter by Location" box to just search for appointments open to either essential or non-essential employees on those specific days. On April 15th and 16th, all appointments are open to both essential and non-essential employees. 

By participating in the INHS health screening, I authorize Jubilant HollisterStier LLC to deduct $20 from my paycheck to cover the cost of my April 2021 health screening and an additional $15 fee if I do not show up.
